Mr. Robert Charleston admitted to Majestic Care New Haven after a recent hospitalization due to a fall and post-COVID affects.
Robert has experienced an overall decline in self-care, transfers, mobility, cognition, and ambulation, requiring extensive assistance with all areas of care. Robert was able to participate in Physical Therapy, Occupational Therapy, and Speech Therapy services provided by Reliant Rehabilitation. Through his diligence and hard work, he was able to regain functional independence in all areas identified on his plan of care.
At the time of discharge Mr. Charleston was demonstrating modified independence with self-care, transfers, and moving without assistance. Robert showed improved cognition and was able to use his strategies to recall appointments and daily activities. Robert safely returned to assisted living. Mr. Charleston was very appreciative of his Therapy team.
