Medical Social Services:

• Work closely with family members and caregivers of patients providing updates on behavior and physical state of their loved one
• Provide psychoeducation and counseling to patients and caregivers to assist with understanding illness and disease processes
• Identify and collaborate with community resources for purpose of addressing ongoing supportive services
• Provide advocacy support on behalf of the patient, family, and caregivers
• Assists the patient with successful transition to outpatient providers for continuity of care.
• Developing and implementing a plan of care designed to meet the patient’s post discharge needs while ensuring continuity of care
• Follow up with outpatient referrals and consults
• Serve as an integral interdisciplinary team member by collaborating with physicians and other disciplines to develop a plan of care
• Work closely with facility discharge planners while communicating with families and facilities during a patient’s discharge process

 

Occupational Therapy:

  • Develop homemaking skills
  • Teach activities for daily living
  • Develop home safety strategies
  • Developing individuals for maximum independence
  • Cooperatively working to have the proper adaptation of individual’s work/living conditions
  • Strength, coordination & endurance development

Speech Therapy:

  • Cognitive retraining
  • Dysphagia/Swallowing
  • Communicative disorders

Skilled Nursing:

We offer a full range of care services including everything from Skilled Nursing to Physical Therapy to Speech Therapy to Memory Care.
