Paul Estala was admitted to Majestic Care of New Haven after a declining ability to care for himself, resulted in hospitalization.

Paul was admitted with an overall decline in self-care, transfers, functional mobility, and ambulation. He required extensive assistance within all areas of care. Through Reliant Rehabilitation, physical therapy, and occupational therapy, a plan of care was developed to address his goal of transitioning to an assisted living facility. 

Paul worked hard with physical therapy and occupational therapy to regain his functional independence and was able to transition into the community to an assisted living facility.
