Marta Kohen chose to call Majestic Care of Avon home. While at her previous facility, Marta was a high fall risk due to decreased balance, decreased strength and declining mobility. Upon arrival to Majestic Care of Avon, Marta was evaluated by Reliant Rehabilitation’s Physical Therapy Department.
Marta was motivated in improving her current and regaining independence in her new environment. She participated in therapy 5 days a week, going above and beyond during her treatment sessions. Physical therapy’s main goal was to make Marta independent again, decrease her fall risk and promote her independence in her new environment.
She completed therapy earning the level of modified independence. Marta is able to independently ambulate to all meals and she attends activities and assists with other residents. Marta has become more confident and outgoing due to her improved health.
Marta feels without therapy she would not have transitioned as well to her new environment. “I could have fallen and broke something. I feel like now I have the strength to do anything I want.”
