Brian was admitted to our community for rehabilitation, dialysis treatment, and a possible long-term placement. Thanks to our amazing therapy team with Reliant and Brian’s hard work and determination, he was able to discharge to his own apartment in assisted living! Congratulations to Brian and his Care Team on their success! Bryan says, “I had a good experience there (Jefferson Pointe) with dialysis. It was close to my room, and the facility was really beautiful.”
