James Urban was admitted to Majestic Care of New Haven after he was recently hospitalized from multiple falls at home. When James first arrived, he required increased assistance with self-care, mobility, transfers, and cognition. A plan of care was developed to address his goals of regaining independence and returning back home with his wife. James worked diligently with therapy services to increase his strength, activity tolerance, self-care skills, and safety awareness to reduce his risk for falls when returning home.
