We’re excited to share this inspiring story of resident, Mr. Eugene Mitchell.

Mr. Eugene Mitchell experienced declining physical condition for several months while living on his own. During this time Eugene had multiple hospitalizations and short stays at various rehabilitation centers. 

Mr. Mitchell was admitted to Majestic Care of New Haven on January 21, 2021, for complications associated with diabetes.  He required extensive service with cognition, self care, transfers, and mobility due to weight-bearing restriction of his left foot. A plan of care was developed by physical therapy, occupational therapy, and speech therapy through Reliant Rehabilitation to address his goals of increasing independence in self-care and transfers, and ultimately returning to the community with the support of his family. 

Mr. Mitchell’s diligence and dedication with his therapy enabled him to increase his overall strength, balance and activity tolerance to increase functional independence.  At the time of discharge, Eugene demonstrated independence in self-care at wheelchair level, improved cognition and independence in transfers. 

On February 21, 2021, Eugene safely returned home with the support of his family and community resources. Mr. Mitchell was very appreciative of his Therapy team and all the staff at Majestic Care of New Haven.
