Majestic Care of Jefferson Pointe is proud to share the success story of our resident, Patricia Wooten!

Patricia was admitted following a fall at home, returning as a familiar patient after previously receiving rehab and dialysis care with us for a hip replacement. This time, she faced a new fracture that left her non-weightbearing on her left leg. During her stay, Patricia received in-house dialysis alongside comprehensive rehabilitation services. Her Speech Therapy focused on safety awareness and problem-solving skills, preparing her for an independent return home with minimal home health support. Occupational Therapy helped Patricia regain the ability to perform activities of daily living, progressing from needing assistance to modified independence! Physical Therapy worked diligently to improve her lower body strength, balance, transfers, and ambulation.

Upon discharge, Patricia was able to ambulate over 150 feet, transfer independently, and complete bed mobility without assistance! Thanks to a coordinated team effort involving nursing, dialysis, and therapy, Patricia successfully returned home to Pennsylvania stronger, safer, and more independent. Congratulations to Patricia and her Care Team on their success!
